With age, skin loses density and elasticity not only on the face but also in the intimate area. Perimenopausal patients often report frequent vaginal infections, associated not only with vaginal mucosal atrophy and decreased levels of sex hormones but also with atrophy of the labia majora, which forms a natural barrier to protect the vaginal entrance.

Vaginal dryness, affecting approximately 20 percent of women, often leads to discomfort. Labial atrophy results from the loss or reduction of subcutaneous fat in this area, which can be due to menopause or genetic factors. Women engaged in sports like horseback riding or cycling often experience the most discomfort, as this area can become painful.

Another, albeit less common, indication for this procedure is significant labial asymmetry, which may cause psychological distress. This procedure is often combined with labiaplasty of the labia minora to achieve a better cosmetic outcome.

Hyaluronic acid injections restore the natural volume and moisture of the skin. Additionally, the procedure stimulates collagen synthesis in fibroblasts due to mechanical pressure. The treatment involves disinfecting the vulvar area and applying local anesthesia to the injection sites. Following a needle puncture, the cannula is introduced into the subcutaneous tissue of the labia majora, typically with around 1 milliliter per side, beginning at the lower pole and advancing symmetrically. The procedure takes approximately 30 minutes and is virtually painless for the patient.

The treatment results in adequate hydration of the dermis’s middle and deep layers, increased volume, and restored labial symmetry. The benefit is that the effect is immediate and noticeable to the patient, intensifying over the next two weeks as water absorption occurs. Typically, repeat treatments are unnecessary for 1-2 years. Post-procedural restrictions are minimal; the patient should avoid pressure on the perineum for about two weeks, including refraining from activities like horseback riding, cycling, or wearing tight clothing.
