Your Med Medical Center

I. General Provisions

1. Purpose of the Regulations: These regulations define the terms of service provided by Your Med Medical Center, located in Warsaw at Magazynowa 9/13 street, 02-652 Warsaw, and regulate the rights and obligations of patients and medical staff.

2. Scope of Applicability: These regulations apply to all patients and individuals using the services of Your Med Medical Center.

3. Service Provision Guidelines: Medical services are provided in accordance with applicable laws, particularly the Act on Medical Activity, patient rights, and medical standards.

II. Registration and Appointment Scheduling

1. Registration Methods: Patients may register in person, by phone, via the online reservation system available on the Your Med Medical Center website, or through the Znany Lekarz service.

2. Opening Hours: Your Med Medical Center is open Monday to Friday from 08:00 to 21:00 and selected Saturdays from 10:00 to 15:00.

3. Appointment Confirmation: Patients are required to confirm their appointments at least 24 hours before the scheduled time. Failure to confirm may result in the cancellation of the reservation.

4. Appointment Cancellation: In case of cancellation, patients should notify Your Med Medical Center at least 24 hours in advance. If the appointment is not canceled on time, a fee of 300 PLN may be charged.

 

III. Terms of Using Medical Services

1. Patient Rights: Patients have the right to professional and courteous service, protection of personal data and information concerning their health status, and comprehensive information about medical procedures and treatments.

2. Patient Responsibilities:

◦ Attend scheduled appointments on time.
◦ Provide complete and accurate information regarding their health, medical history, and medications.
◦ Follow recommendations from doctors and medical staff.

3. Appointment Lateness: If a patient is more than 15 minutes late, the doctor may shorten the consultation time or reschedule the appointment.

4. Medical Documentation:Patients have the right to review their medical records and request copies. Documentation may require a written request.

 

IV. Service Fees

1. Service Price List: Fees for medical services are specified in the price list available at the Your Med Medical Center reception and on the website. The price list is subject to change, and patients will be notified in advance.

2. Payment Methods: Payments can be made in cash, by credit card, in accordance with arrangements made during registration.

3. Additional Fees: In cases where appointments are not canceled in a timely manner, Your Med Medical Center may charge a fee of 300 PLN for the appointment.

V. Visit Procedure

1. Patient Registration: Patients are required to check in at least 10 minutes before their scheduled appointment to confirm their details and complete formalities.

2. Medical Procedure: The doctor or other medical professional presents the patient with the treatment plan, possible procedures, and any associated risks.

3. Consent for Procedure: Before undergoing invasive procedures, patients must provide written consent for the procedure.

VI. Data Protection

1. Data Processing Principles: Patients’ personal data are processed in accordance with the Personal Data Protection Act and the privacy policy of Your Med Medical Center.

2. Purpose of Data Processing: Patients’ personal data are processed solely for the purposes of providing medical services, managing medical records, and maintaining contact with the patient.

3. Data Access: Patients have the right to access their personal data, request corrections, or deletion of data at any time, in accordance with applicable law.

VII. Patient Rights and Responsibilities

1. Right to Dignified Care: Patients have the right to respect, dignity, and reliable information about their health status and available treatment methods.

2. Right to Confidentiality: All information regarding patients, including their health status, is subject to medical confidentiality and is processed in accordance with applicable laws.

3. Patient Responsibilities:

Follow the doctor’s recommendations and instructions from medical staff.
◦ Adhere to the rules and standards of Your Med Medical Center, including maintaining cleanliness and respecting property.

VIII. Complaints

1. Complaint Procedure: Patients have the right to file complaints regarding the services provided, quality of service, and other aspects of the activities of Your Med Medical Center.

2. Complaint Handling Procedure: Complaints can be submitted in writing at the reception desk or via email at yourmed@wp.pl. Your Med Medical Center undertakes to respond to complaints within 14 working days.

IX. Final Provisions

1. Amendments to the Regulations: Your Med Medical Center reserves the right to make changes to these regulations. Changes will be published on the website and available at the reception.

2. Effective Date: These regulations take effect from January 1, 2020, and are valid until amended or updated

Language Clause

These regulations have been prepared in both Polish and English versions. In the event of any discrepancies between the two versions, the Polish version shall be deemed binding.
